dc.description.abstractABSTRACT Horizontal root fractures represent a clinical challenge as they affect anterior teeth, compromising esthetics, function, and the patient’s quality of life. Although relatively uncommon, accurate diagnosis and multidisciplinary treatment planning are essential for tooth preservation and therapeutic success. This case report describes the clinical management of a 19-year-old male patient, victim of a motorcycle accident, who presented a horizontal root fracture in the maxillary left lateral incisor. The adopted protocol involved surgical extrusion of the remaining root, followed by endodontic treatment using the single-cone obturation technique and subsequent cementation of a glass fiber post with dual-cure resin cement. The restorative phase included coronal reconstruction with incremental composite resin, aided by a silicone guide obtained from a diagnostic wax-up, which ensured greater esthetic and functional predictability. After finishing and polishing, the restoration achieved satisfactory esthetic integration with adjacent teeth and functional recovery, along with the onset of periapical bone neoformation consistent with physiological repair. This case highlights that a conservative approach integrating Endodontics, Restorative Dentistry, and Esthetics is a predictable and effective alternative for preserving anterior teeth affected by horizontal fractures, providing functional and esthetic results and restoring patient self-esteem.
dc.description.resumoAs fraturas radiculares horizontais representam um desafio clínico relevante por afetarem dentes anteriores, comprometendo estética, função e qualidade de vida do paciente. Embora sua ocorrência seja relativamente rara, o correto diagnóstico e o planejamento terapêutico multidisciplinar são fundamentais para a preservação do elemento dental e para o sucesso do tratamento. O presente relato descreve o manejo clínico de um paciente de 19 anos, vítima de trauma motociclístico, que apresentou fratura horizontal radicular no incisivo lateral superior esquerdo (elemento 22). O protocolo adotado envolveu a extrusão cirúrgica do remanescente radicular, seguida de tratamento endodôntico com obturação pela técnica do cone único e posterior cimentação de pino de fibra de vidro com cimento resinoso dual. A fase restauradora incluiu reconstrução coronária com resina composta em técnica incremental, auxiliada por guia de silicone de condensação obtida a partir de enceramento diagnóstico, o que possibilitou maior previsibilidade estética e funcional. Após acabamento e polimento, observou-se integração estética satisfatória com os dentes adjacentes e restabelecimento da função mastigatória e fonética, associado ao início de neoformação óssea periapical compatível com processo de reparo fisiológico. O caso relatado evidencia que a abordagem conservadora, integrando Endodontia, Dentística Restauradora e Estética, constitui alternativa previsível e eficaz para a preservação de dentes anteriores com fraturas horizontais, proporcionando resultados estéticos e funcionais favoráveis e devolvendo autoestima ao paciente
