dc.description.abstractCondensing osteitis (CO) is a type of osteomyelitis characterized by areas of localized bone sclerosis at the apices of teeth with chronic pulpitis or necrotic pulps. It presents radiographically as a radiopaque lesion, where there is a localized bone reaction from a low- grade inflammatory stimulus, usually located at the apex of a tooth or at the extraction site where there has been long-standing pulp disease. Bruxism is a parafunction related to muscle hyperactivity, with a multifactorial etiology, and can be classified as a non-spontaneous act, leading to disordered jaw movements, generating clenching and/or grinding of the teeth, thus overloading the stomatognathic system. This report describes a 35-year-old Caucasian female patient complaining of severe headache. During the medical history, she mentioned being a bruxist. Clinical examination revealed left-sided mandibular interference, returning to the midline, and potential disc displacement with ipsilateral reduction. During protrusion, the disc deviated to the left. Regarding vestibulocochlear symptoms, she presented tinnitus in the right ear and ear fullness in both ears, as well as occasional vertigo. Physical examination revealed a clicking sound during opening movements in the right temporomandibular joint (TMJ). Sensitivity to palpation of muscle trigger points was observed, and marked tenderness was observed on the right side. Radiographic examination revealed large radiopaque areas located at the apices of teeth 36 and 46, where there were extensive amalgam restorations, which were endodontically treated after chronic pulpitis was found in both. The patient is undergoing treatment for cervicomastigatory dysfunction and related bruxism, which, in theory, was the aggravating factor for the bilateral OC in the mandibular molars.
dc.description.resumoA osteíte condensante (OC) é um tipo de osteomielite que se caracteriza por áreas de esclerose óssea localizadas, relacionadas aos ápices dos dentes com pulpite crônicas ou polpas necrosadas. Apresenta-se radiograficamente como uma lesão radiopaca, onde há uma reação óssea localizada a partir de um estímulo inflamatório de baixo grau, geralmente localizada no ápice de um dente ou no local de exodontia, onde houve uma doença pulpar de longa data. O bruxismo é uma parafunção que tem relação com a hiperatividade muscular, com etiologia multifatorial, podendo ser classificado como um ato não espontâneo, levando a movimentos desordenados na mandíbula gerando apertamento e/ou ranger de dentes, causando assim uma sobrecarga no sistema estomatognático. O presente relato relata trata- se de paciente sexo feminino, leucoderma, 35 anos, com queixa de cefaleia intensa. Durante a anamnese mencionou ser bruxista. Ao exame clínico foi verificada interferência mandibular para o lado esquerdo com retorno a linha média com potencial deslocamento de disco com redução ipsilateral. Em protrusão ela desviava para esquerda. Quanto a presença de sintomatologia vestibulococlear, apresentava sintoma de zumbido (tinnitus) na orelha direita e plenitude auricular em ambas as orelhas, além de vertigem ocasional. Ao exame físico apresentou estalido durante movimento de abertura na Articulação Temporomandibular (ATM) direita. Relativo à sensibilidade à palpação de pontos gatilhos musculares observou-se grande sensibilidade do lado direito. Ao exame radiográfico observou-se grandes áreas radiopacas localizadas nos ápices dos dentes 36 e 46, onde havia restaurações extensas em amalgama, sendo tratados endodonticamente, após a constatação de pulpite crônica em ambos. A paciente encontra-se em tratamento da disfunção cervicomastigatória e do bruxismo relacionado, que, em tese foi o fator agravador da OC presente bilateralmente nos molares mandibulares.
