Resumo

This research aimed to evaluate the orientation of oral health through the actions of the National Oral Health Policy – ​​Brazil Smiling – in the context of the State of Amazonas. Furthermore, it sought to assess oral health coverage and access to dental services, analyze data related to the growth of oral health teams and dental services provided to the state's population, and evaluate access to and effectiveness of dental services in primary and specialized care in the Purus and Juruá health regions. This was a retrospective and descriptive study with a quantitative approach. Primary and secondary data sources were used. Primary data were collected through a questionnaire with 16 closed questions, applied to dentists in the Purus and Juruá health regions.Secondary data were extracted through information systems, comprising indicators in Primary Care of coverage of the first scheduled dental consultation and collective supervised tooth brushing action. To evaluate Specialized Care, the indicators adopted were the absolute number of periodontal treatment procedures, the number of endodontic treatments, and the number of dental prostheses performed. The data were analyzed using descriptive statistics and organized into tables in Microsoft Excel®. As main findings, the expansion of oral health teams in Amazonas was observed, with a growth of 532.69% between 2004 and 2024, with an increase in the coverage of the Family Health Strategy from 28.5% to 48.5%.However, primary care indicators suffered a sharp decline from 2015 onwards, with a slight recovery in 2023. Prosthesis production showed growth, albeit with fluctuations, unlike periodontics and endodontics procedures, which registered a reduction in recent years. The analysis in the two health regions revealed inequalities; Purus showed more organized access (50.9% with scheduled appointments) and greater effectiveness (62.2% with specialist consultations and 70.3% with matrix support), while Juruá had more fragmented access (60% by spontaneous demand), absence of Dental Specialties Centers (CEOs), and a low supply of specialties. The results revealed that, despite the structural advances promoted by the National Oral Health Policy (PNSB), challenges related to access, continuity of care, and effectiveness of dental services in the state of Amazonas still persist. This reinforces the need for continuous monitoring studies of the National Oral Health Policy, considering the different regional specificities.
