Abordagens clínicas no uso de materiais biocerâmicos em endodontia: relato de caso

Resumo

The advancement of biomaterials in endodontics has provided more biocompatible and effective alternatives for treating teeth with pulpal compromise and periapical lesions. Among these innovations, bioceramic materials stand out for their superior physicochemical and biological properties, promoting better sealing, regenerative potential, and prolonged antimicrobial effect. This study aims to report a clinical case of endodontic treatment using bioceramic intracanal medication and sealer, evaluating clinical and radiographic outcomes over six months. A 27-year-old male patient attended the POUEA in March 2025, within the extension project Trauma Zero, reporting sensitivity in the upper central incisors due to previous dental trauma. Clinical and tomographic examinations revealed periapical lesions in teeth 11 and 21 and canal calcification. The endodontic access of tooth 11 was performed using a clinical microscope and ultrasonic inserts due to extensive calcification and an 8 mm distance to the pulp chamber, allowing a conservative and precise approach with maximum preservation of dental structure. In contrast, access in tooth 21 was performed conventionally, as the canal presented greater patency and a lower degree of obliteration, making magnification unnecessary. This difference highlights the importance of individualizing clinical protocols according to canal anatomy and calcification level. Treatment included chemomechanical preparation, intracanal medication with Bio-C Temp® (Angelus, Londrina-PR), and obturation with bioceramic sealer Bio-C Sealer® (Angelus, Londrina-PR) using the single cone technique. Follow-up showed satisfactory evolution, with periapical healing and absence of active inflammation. The patient reported 70–80% pain reduction, maintaining mild occasional sensitivity during chewing. These results confirm the bioactive potential of bioceramic materials, which promote an alkaline environment, sustained calcium and hydroxyl ion release, and formation of an apatite-like layer at the material-dentin interface, stimulating tissue repair. It is concluded that the combined use of Bio-C Temp® and Bio-C Sealer® proved to be effective and safe, representing a promising alternative in contemporary endodontic practice.
