Diagnóstico de carcinoma espinocelular moderadamente diferenciado HPV associado em paciente senil: relato de caso

| dc.description.abstract | Squamous cell carcinoma (SCC) is a common malignancy in the head and neck region, accounting for more than 90% of cases of oral malignancies. Human papillomavirus (HPV) is a double-stranded virus belonging to the papillomaviridae family that lives in the cutaneous and mucosal epithelium due to its specific tropism for epithelium. More than 200 types of HPV have been described, but not all are related to oral cavity disease. High-risk HPV types are associated with dysplasia and SCC, especially HPV16 and 18. The objective of this study is to report the diagnosis of moderately differentiated SCC associated with HPV in a senile patient, with the aid of immunohistochemistry, since histopathological examination alone was not sufficient to confirm the viral etiology of the malignancy. A 67-year-old female patient was referred to the POUEA stomatology clinic, presenting clinically with a leukoerythroplastic lesion of lichenoid pattern measuring approximately 2 cm in the anterior 2/3 of the right lateral border of the tongue, with an ulcer in the center. An incisional biopsy was performed and the histopathological report showed moderately differentiated SCC. Immunohistochemical analysis was recommended, where the biomarkers PanCK, P16 and P53 tested positive and Ki-67 had a rate of 40%, which certified HPV as the etiological agent of the malignancy. | |
| dc.description.resumo | RESUMO O carcinoma espinocelular (CEC) é uma malignidade comum na região de cabeça e pescoço. Representando mais de 90% dos casos de neoplasias malignas orais. O papilomavírus humano (HPV) é um vírus pertencente a família papillomaviridae que vive no epitélio cutâneo e mucoso devido seu tropismo específico por epitélio. São descritos mais de 200 tipos de HPV, mas nem todos estão relacionados a doença da cavidade bucal. Os tipos de HPV de alto risco estão associados a displasia e CEC, com destaque aos HPV16 e 18. O objetivo desse trabalho é relatar o diagnóstico de CEC moderadamente diferenciado associado ao HPV em uma paciente senil, com o auxílio da imunohistoquimica, visto que somente com o exame histopatológicos não foi o suficiente para confirmação da etiologia viral na malignidade. Paciente do sexo feminino,67 anos foi encaminhada a POUEA para clínica de estomatologia apresentando clinicamente uma lesão leucoeritroplásica de padrão liquenoide de aproximadamente 2cm nos 2/3 anteriores de borda lateral direita da língua, com uma úlcera no centro. Foi realizado biópsia incisional e o laudo histopatológico teve como resultado CEC moderadamente diferenciado. Foi recomendado análise imunohisto-química, onde P16 e P53 testaram positivo, além de PanCK e Ki-67 que teve uma taxa de 40%, a positividade para os dois primeiros certificou o HPV como agente etiológico da malignidade. | |
