Resumo

The objective of this graduation thesis is to report a case of cutaneous tuberculosis involving the stomatognathic system, specifically in the lower lip. A 51-year-old female patient with melanoderma, from Manaus, sought treatment at the Odontological Polyclinic of UEA, in the Stomatology Clinic, due to an ulcerative lesion on the lower lip, with hardened margins, associated with edema and sensitivity. An excisional biopsy was indicated, and the histopathological examination revealed granulomatous formations composed of macrophages and multinucleated giant cells in the center of the inflammatory response. Initially, the diagnostic hypothesis of Chagas disease was raised, as amastigote and epimastigote forms of Trypanosoma cruzi were observed. However, imaging tests (chest X- ray and computed tomography), associated with a new biopsy and additional laboratory tests, confirmed the diagnosis of pulmonary and cutaneous tuberculosis, ruling out the possibility of Chagas disease. The case emphasizes the importance of proper differential diagnosis in ulcerated lesions of the lip and highlights the need for a multidisciplinary approach for correct clinical management, allowing for better prognosis and quality of life for the patient.
