dc.description.abstractThe replacement of a dental implant is indicated in cases of failure osseointegration, aesthetic or functional dissatisfaction, peri-implant infections, and fractures. This replacement is often associated with the need for bone and connective tissue grafting, na association that plays a key role in ensuring aesthetic, functionality, and natural appearence. The aim of this study is to report a clinical case of replacement of an external hexagon implant with incorrect three- dimensional positioning in the anterior maxillary region at thoot 12, accompanied by the absence of vestibular bone tab, describing the therapeutic approach adopted, which includes removal of the implant using the reverse torque technique, with the aid of a ratchet and retrivier, followed by replacement with a Morse taper model, as well as particulate xenogeneic bone graft and autogenous connective tissue. The 50-year-old patient presented with an unsatisfactory aesthetic and functional condition. Imaging revealed incorrect three-dimensional positioning associated with bone plate destruction and tissue loss, which were decisive factors in the decision-making process.
dc.description.resumoA substituição de um implante dentário é indicada em casos de falha na osseointegração, insatisfação estética ou funcional, infecções peri-implantares e em casos de fraturas. Muitas vezes essa substituição está associada a necessidade de enxerto ósseo e conjuntivo, associação essa que tem um papel primordial na estética e funcionalidade, garantindo a naturalidade. O objetivo deste trabalho é relatar um caso clínico de substituição de implante hexágono externo com posicionamento tridimensional incorreto na região anterior da maxila no elemento 12 e ausência de tabua óssea vestibular, descrevendo a abordagem terapêutica adotada, que inclui a remoção do implante através da técnica do torque reverso, com auxílio do retrivier, seguido de substituição por um do modelo cone morse, assim como enxerto ósseo xenogéno particulado e conjuntivo autógeno. A paciente, de 50 anos, apresenta condição estética e funcional insatisfatória. No exame de imagem, foi possível observar um posicionamento tridimensional incorreto associado destruição da tabua óssea e perda tecidual, esses fatores foram decisivos para a tomada de decisão.
