Item Achados laringoscópicos em pacientes com artrite reumatóide em um serviço de referência em otorrinolaringologia da região norte(Universidade do Estado do Amazonas, 2019-01-18) Oliveira, Viviane Saldanha; Monteiro, Mariana Raposo Alencar; Siqueira, Gisele Maia; Mafra, Mayara; Delfino, Ana Carolina Guimarães; Soares, Alex Wilker Alves; Pryjma, Alexandre Augusto LeãoRheumatoid arthritis (RA) is an autoimmune disease that affects small and large joints, which may also manifest in the larynx. Objective: To study the laryngeal manifestations in patients with rheumatoid arthritis treated at an otolaryngology referral service of Manaus. This is a cross-sectional prospective study with clinical assessment and laryngoscopy of patients with rheumatoid arthritis. We studied 43 patients, of whom 81.4% were female. The main complaints were laryngeal dysphonia in 76.8% and 32.6% in pharyngeal globus. The most common videolaryngoscopic finding was the posterior laryngitis found in 97.7%. Possible findings related to RA were the posterior laryngitis and vocal fold nodule. The posterior laryngitis was the main finding and may be associated with RA and / or secondary pharyngolaryngeal reflux to drug treatment for rheumatoid arthritis. Keywords: Rheumatoid arthritis; larynx; vocal cords; arytenoid.Item Análise quantitativa dos antígenos de superfície do vírus da hepatite B em portadores de hepatite B em associação com vírus da hepatite D no Amazonas(Universidade do Estado do Amazonas, 2019-01-21) Araújo, Ana Ruth Silva de; Souza, Anthony Reis Mello de; Lira, Edson da Fonseca de; Novais, Maria Auxiliadora MonteiroThis is a study to examine the HBsAg levels in patients with Hepatitis B in association with the hepatitis D virus in the Amazon Background & Aims. There are an estimated 400 million chronic carriers of hepatitis B virus in the world, and in Brazil, mostly in the amazon region with 5 to 15% prevalence of HBV infection, and 15-20 million of these individuals may be co-infected or super-infected with HDV. Studies suggest quantitative HBsAg in combination with HBV-DNA as a diagnostic tool, prognostic and a predictive marker for antiviral therapy. It is a cross-sectional study, a descriptive case series evaluating the concentrations of quantitative HBsAg in the natural history of patients with HBV infection in association with HDV. We analyzed demographic data and laboratory tests performed to determine the concentrations of ALT, viral load, HBeAg status and HBsAg quantitative of 48 patients. There was no difference between the sexes. There was a predominance of young adults. The stage of the disease had a higher incidence was low replication, and immune tolerance phase that was held the highest average quantity of HBsAg. There is statistically significant association between the HBeAg and anti-HDV. There were more among HBeAg-negative patients co-infected than in mono-infected. There was no statistically significant association between quantitative HBsAg and ALT, HBsAg quantitative and viral load and HBsAg quantitative and HBeAg status. There is a significant association between anti-HDV. and HBV viral load levels. The HBsAg quantitative is higher in the phase of immunotolerance. There is high prevalence of co-infection HBV/HDV in the state of Amazonas. This disease seems to be related to inhibition of HBV replication and the quantitative HBsAg level. Depending on its size and location, it may lead the patient to have criteria for a difficult airway. Objective: to report the case of a patient with a difficult airway due the presence of a myopephelioma in the hard palate region, this way showing the importance of a good anesthetic planning in the face of difficult airways, obtaining it in a definitive and safe way. Method and theoretical basis: For the achievement of the procedure, a previous evaluation of the airway was executed through laryngoscopy, after superior and recurrent laryngeal nerve blocks, followed by definitive nasofibroscopy, considered the gold standard method for difficult airway management. Having as theoretical basis a set of papers related to the subject, such as: Lukte Claudia, Robin Stackhouse, Jeffrey L. Apfelbaum, Carin A. Hagberg, C. Frerk, V.S. Mitchell. Results: Appropriate anesthetic planning through the evaluation and careful approach, with appropriate techniques and materials, of the airway in a patient with difficult airway, as in the case reported above, tends to reach the expected positive result. In the case reported, the patient was evaluated and subsequently underwent general anestesia, the nasotracheal intubation happening without adverse events reports, the surgical procedure also following without any incidents.Keywords: Difficult air route; Myoepithelioma; Anesthesia.Item Angioedema hereditário: uma doença subdiagnosticada(Universidade do Estado do Amazonas, 2019-01-21) Rodrigues, Lorena R. C.Hereditary angioedema is an autosomal dominant disorder with a prevalence of 1: 10.000-50.000 people, relatively rare, due to genetic mutations that determine qualitative or quantitative deficiency of C1 esterase inhibitor (C1-INH). The causes and pathophysiological mechanisms of angioedema are diverse, existing acquired forms and, more rarely, hereditary. The low concentration of C1-INH results in disordered of various systems such as complement, kinin and coagulation with the formation of vasoactive substances. The sites most affected are the face, extremities, genitals, oropharynx, larynx and system digestivo. Drug treatment can be divided into three categories: long-term prevention, short-term treatment and prevention of crises. This study aims to determine the importance of early diagnosis by health professionals based on the clinical picture, family history and complement the study, whereas the Hereditary Angioedema is potentially fatal and requires specific therapy. Given the widespread use among individuals who practice physical activities, regardless of regularity, of dietary supplements, need for characterization of consumption with knowledge of the main products and influences for the beginning of their use, and the establishment of the profile of these consumers in the city of Manaus, Amazonas; aiming improvement in sports performance and nutrition, with consequent orientation of this square, avoiding losses that at times may be irreversibles. It is a descriptive, quantitative study, conducted through questionnaires given to 250 individuals in two Fitness Centers located in the city of Manaus, who said they were in use of ergogenic. It identified a majority to the male gender 62% (155), with mean age of 28 years. The source of indication most cited were the instructors and teachers of physical education, followed by sales of products, friends and bootstrap. Furthermore, the main consumption targets were whey Protein (73%) and branched chain amino acids “BCAA” (79%), occasionally simultaneously use. It feature the everyday use of these supplements, mostly without the accompaniment of qualified professional, with possibility of serious health problems, warning of a current and serious problem. Objective: evaluate the prevalence of changes in the metabolism of carbohydrates in tinnitus patients treated at a referral center for ENT and cervicofacial surgery in the city of Manaus. This is an observational, cross-sectional and prospective study of application and analysis of data from forms of patients with tinnitus. We evaluated 21 patients with tinnitus and 52.4% female. The patients had between 31 (minimum age) and 86 years (maximum age), with a mean of 53. The most frequent buzzing sound descriptions were wheezing (33.3%), zum (14.3%) and whistle ( 14.3%). Of assessed, 28.6% reported bilateral tinnitus with greater intensity on the left, 28.6% were suggestive of blood glucose diabetes mellitus and 9.5% had impaired glucose tolerance. It was found that 38.1% of patients showed blood glucose above the normal value, and in 28.6% values were suggestive of type 2 diabetes mellitus. In osseous coalition, the carpal elements are united as a single osseous block, whereas, in nonosseous coalition, the affected carpal bones are united by either cartilage (synchondrosis), fibrous tissue (syndesmosis), or some combination. Case reports: Case 1: 56 year-old woman with pain and functional limitation in the right wrist for 4 months. It refers to rheumatoid arthritis for 25 years. Case 2: A 39- year-old man with pain in his right hand after trauma for five years. He refers to pain in the right hand before the trauma, but with marked worsening after the episode, limiting the movement of the joint. Discussion: Pancarpal coalition is the coalition of all or most of the carpal bones, when observed, usually occurs within the constellation of one syndrome, like Ellisevan Creveld syndrome, symphalangism, Steel syndrome,Holt-Oram syndrome, arthrogryposis multiplex congenital, otopalatodigital syndrome, and Apert syndrome. The carpal coalition may be possibly related to inflammatory arthropathies - rheumatoid arthritis, juvenile arthritis, psoriatic arthritis and Reiter Syndrome. Conclusion: We report two rare cases of pancarpal coalition of different etiologies documented by radiography and magnetic resonance imaging.Item Correlação entre força muscular respiratória e gravidade clínica em adultos com asma brônquica(Universidade do Estado do Amazonas, 2019-01-18) Salame, Flavia Matos; Moreira, Maria Angela Fontoura; Tesser, LucianaThe measurement of respiratory muscle strength has a wide application, since it allows the diagnosis of respiratory failure due to muscle failure; early detection of respiratory muscle weakness. Objective: The objective of the present study was to evaluate the strength of contraction of the respiratory muscles in asthmatic adults by measuring maximum respiratory pressures and to verify if there is a direct / progressive correlation between worsening respiratory muscle strength and asthma severity (spirometric classification) . Methods: Cross-sectional observational study with a sample composed of outpatients aged between 18 and 60 years with previously confirmed diagnosis of asthma. Spirometric values, PImax and MEP were determined. Three reproducible measures were selected (variation <10%) and there was a decrease in muscle strength related to asthma severity. Results: 124 patients with asthma were evaluated. Separating them by gravity (spirometric classification) we have 24 normal, 42 with mild obstructive ventilatory disorder, 36 with DVOM and 22 with DVOG. There was a correlation between spirometry variables with PImax and MEP significantly in the DVOG group alone. There was an improvement in PEmax and PImax values after bronchodilator testing. Conclusions: In the present sample, the presence of asthma determined significant changes in respiratory muscle strength only in patients classified as severe. There is a very weak linear correlation between worsening respiratory muscle strength and asthma severity. keywords: asthma; respiration muscles; respirations function testsItem Crise tireotóxica como diagnóstico diferencial de doença psiquiátrica: relato de caso(Universidade do Estado do Amazonas, 2019-01-18) Macedo, Viviane Alves de Carvalho França de; Hoshi, YumiThyroid storm is a rare condition, which reflects an extreme physiological state within the thyrotoxicosis spectrum. This compromises various body systems, including the central nervous system. Although the changes in mood and behavior can occur due to thyrotoxicosis, severe neuropsychiatric characteristics such as psychosis, seizures or coma are rarely found. The aim of this study was to show a case report of a patient, 41 years that started with sudden agitation, tachylalia and religious delusion. It was previously healthy with no comorbidities. Clinical and laboratory findings (TSH suppressed and T4> 6) suggested Thyroid storm, scoring 55 points for the diagnostic criteria by Burch and Wartoksky (temperature: 37.7; psychosis, heart rate: 135; precipitating factor). The pathophysiology of neuropsychiatric symptoms in thyroid storm is still unclear and some theories are proposed. The neurological irreversible even with the institution of appropriate drug therapy can happen as evidenced in the patient of this case report, even though he had received drug treatment recommended by the literature, she remained with neuropsychiatric manifestations, although other signs and symptoms of thyroid storm had It has been eliminated. Tetralogy of Fallot is the most common cyanotic heart disease characterized by ventricular septal defect, pulmonary stenosis, biventricular origin of aortic valve and right ventricular hypertrophy. The diagnosis is usually performed in the first year of life due to early clinical manifestations in most cases. However, in some cases, the association of other cardiac defects and failures in the health system can contribute to late diagnosis, increasing the likelihood of unfavorable prognostics. Objective: To report a case of an adolescent patient with late diagnosis of tetralogy of Fallot, with systemic complications of cyanosis. It was surgically treated, evolving satisfactorily. Conclusion: Despite the scientific and technological advances in medicine, it is still possible to identify people at older age and with severe comorbidities not diagnosed, as congenital heart defects, resulting in complications or even death if not diagnosed on time. This fact corroborates the need for awareness of health teams for the early identification of suspected cases. Adult worms can survive in humans permanently for years, in a process known as autoinfection. However, a deregulation of the immune response of the host during latent infection can be lethal, because multiple infective larvae can develop and invade organs other than the intestines, causing sepsis and life. The clinical presentation may vary from strongyloidiasis the indolent a condition life-threatening. Symptomatic autoinfection with epigastric pain and diarrhea occur in people living in endemic areas and those with malnutrition, alcoholism, diabetes or any imunodeficiência. The diagnosis of strongyloidiasis requires a high degree of suspicion, because most patients with the infection do not show characteristics distinct clinical and laboratory and imaging findings often turn out to be inespecíficos. Ivermectin.Item Estudo Epidemiológico dos pacientes submetidos à videolaringoscopia durante a Campanha da Voz no Amazonas(Universidade do Estado do Amazonas, 2019-08-15) Müller, Layla CalazansIntroduction: Voice is one of the main ways of human communication and its inappropriate use can lead to organic and functional lesions of the vocal tract. In this context, the voice campaign comes as an important instrument of primary care. Objective: To characterize the epidemiological profile of patients submitted to videolaryngoscopy in the Voice campaign held in Amazonas from 2012 to 2017. Methods: This is an observational, descriptive and cross-sectional study, analyzing the medical records of patients seen in the voice. Approved on the Local Ethics Committee under CAAE: 69067617.7.0000.5016. Results: Of the 982 patients, 69.04% were female. The mean age of participants was 46.26 years. The main complaint was hoarseness followed by hawking. 66% of the patients had eyelid edema and posterior laryngeal hyperemia. Only 5.8% of the exams did not present alterations. Of these patients, 64.6% were treated for pharyngolaryngeal reflux, 20.7% were referred to the otorhinolaryngologist, 9.7% were to speech therapy and 5.4% were released from the service. Conclusion: The analysis showed a predominance of females and age above 50 years. Hoarseness was the main symptom reported, while the most prevalent finding was posterior laryngeal edema and hyperemia. 1.6% of the patients presented suspected malignancy.Item Estudo placebo-controlado para a avaliação de emagrecimento e compulsão alimentar com fórmula comercialmente disponível de florais de Bach.(Universidade do Estado do Amazonas, 2019-01-18) Tavares, Allan Rafael dos Santos; Maduro, Isolda Prado de Negreiros NogueiraThe objective of this study was to evaluate the potential use of a commercially available “floral for weight loss”, by reducing impulsiveness and binge eating in a group of obese patients. 28 patients were recruited to this prospective, randomized double blind study. They received standardized food plan with 1500 kcal with a replacement list, collected anthropometric data (weight, height, BMI), applied the test for binge eating - Binge Eating Scale (BES), with cutting score ≥17 points to binge eating disorder. They were randomized into 3 groups: a double blind group (floral or placebo) and two tests groups - control (floral and water) receiving respectively "Floral for weight loss" (floral group, n = 7) and vehicle (placebo Group, n = 7); and "Floral for weight loss" (floral test group, n = 7) and water (water control group, n = 7). All groups were treated for 60 days, were excluded patients under 18 years or using drugs to lose weight or to control mood or anxiety. Anthropometric data and BES were repeated at the end of the study. There was significant weight reduction in placebo group (initial weight 94.2 kg ± 9.29 vs final weight 91.2 kg ± 8.9 p = 0.025), but not in the other groups: floral-double-blind (88.1 kg ± 8.3 ± 8.1 vs 87.7 kg, s), floral test (97.4 kg vs. 95.7 kg ± 18.5 ± 95.7 ± 17.7 kg) and water control (85.2 kg ± 7.6 vs 84.3 ± 84.3 kg ± 7.8 kg, ns). The data suggest no association between floral commercially available for weight loss (double-blind / controls) and slimming, although they have not been detected behaviors of binge eating by this instrument. The Kangaroo method (MC) proposes to assist in helping these PN so economical, beneficial and affordable. The objective was to evaluate the weight gain of PN admitted in the third phase of the MC, on motherhood in the period April/2012 until April2014. A retrospective cross-sectional study carried out and approved by the Research Ethics Committee of the Universidade do estado do Amazonas (UEA). Participated in 86 patients, 43% men and 57% were female. The number of pregnancies, previous births, prenatal consultations, as well as gestational age and weight of PN on the admission of the MC, were similar between the groups of PN on exclusive breastfeeding (AME), compared to those who received the supplement on breastfeeding (AMC). Mothers whose PN remained in AME showed significantly lower age compared to those on AMC. However, the follow-up time was significantly less in AME. PN on AMC showed greater weight gain and greater weight on the MC when compared to the group in AME. Gestational age in high, also was statistically higher among those on AMC. As a conclusion it is observed that in the present study, weight gain appears to be greater in PN which received Add-ons on breastfeeding. However the PN receiving exclusive breastfeeding were able to complete the MC, earlier. Keywords:Weight gain, Kangaroo Method and PrematureItem Hipodontia associada à fissura unilateral de lábio e palato em paciente não sindrômico: relato de caso(Universidade do Estado do Amazonas, 2019-02-10) Lima, Marcelo Capistana deHypodontia is a form of tooth agenesis consisting of congenital absence of one to six teeth. Tooth agenesis is frequently described in combination with cleft lip and/or palate (CL/P). The aim of this work is to present a clinical case of hypodontia associated with right sided and complete cleft palate in a 16 years-old melanodermic non-syndromic young male. He was attended at the dentistry service of the Special Patients Center of the University of the State of Amazon. At clinical exam was verified the previous accomplishment of cheiloplasty and palatoplasty, besides decay lesion in the elements 36 and 46. Tooth agenesis of the teeth 25, 18, 38 and 48 was detected at radiographic exam. Restoring procedures were accomplished at the carious elements with oxide of zinc and eugenol and the patient was referred to another health unit to perform the permanent restorations and dental care. A pattern, commonly described for this type of cleft was not observed in this case, demonstrating that a strong and independent genetic basis might play an important role in tooth agenesis, despite the presence of CL/P.Item Histiocitoses de células de Langerhans no Paciente Pediátrico: apresentação de um caso clínico(Universidade do Estado do Amazonas, 2019-01-18) Rocha Júnior, Mario Tércio; Dias, Nayara Alencar; Reis, Monique Freire dos; Maia, Maiko Ramos; Ferreira, Luiz Carlos de Lima; Taveira, Adriana Távora de AlbuquerqueIn the pediatric group histiocytoses are rare diseases with diverse and severe staff in its clinical presentation. To describe a case of Langerhans cell histiocytosis in childhood, more specifically syndromic Letterer-Siwe disease. Case report: Infant, 7 months, male, blotches frame erythematous and pruritic, febrile periods, bulging region BTE bilaterally fibroelastic consistency, fetid otorrhea left, nail lesions and ulcers in ophthalmic mucosa, hepatosplenomegaly, skin jaundice and sclera, tumor increase retroauricular bilaterally evluindo with pnmeumonia worsening hepatic framework cuminando with death. CT chest bilateral sparse pulmonary cysts accompanied by multiple lymph node enlargement mediastinal coalescing. A CT scan of the abdomen hepatomegaly with multiple nodules. Magnetic Resonance Cholangiopancreatography suggestive of congenital hepatic fibrosis associated with fibrocystic changes, saccular dilation. Immunohistochemical evaluation of cervical biopsy was positive for anti-CD68 antibodies, CD1a and S100, and negative for CD30, concluding diagnosis of Langerhans cell histiocytosis, Letterer-Siwe disease. There was no Langerhans cells to liver biopsy. The diagnosis of LCH is a challenge to the pediatrician that requires precision and speed to initiation of therapy, and should be remembered as a differential diagnosis, despite its rare incidence. To determine the incidence of breastfeeding in neonates’ discharge from hospital, following Kangaroo Care Method’s third stage. A Transversal and Retrospective Epidemiological Study conducted with data obtained from newborns’ charts examined at ambulatory care facilities in Kangaroo Care Method’s third stage, at a public maternity hospital in the municipality of Manaus from January up to December, 2015. The sample comprised 550 low-birth weight newborns; 24% of who were of very low birth weight, a high-risk group for early weaning. Differently from literature, the service has a high index of breastfeeding even amongst the very low birth weight newborns. 93.1% of low birth weight babies and 91.6% of newborns weighing less than 1500g were discharged from the ambulatory care facility at Kangaroo’s 3rd stage of exclusive breastfeeding. Although newborns of low birth weight are at increased risk of early weaning, it was found in the sample a high frequency of exclusive breastfeeding at discharge, even among VLBW infants. The Kangaroo Mother care, The Baby- Friendly -Hospital, the work of the Human Milk Bank, compliance with Brazilian Law for Food Marketing Infant Formula, the appreciation of the multidisciplinary team, including composed of international consultant in lactation, are strategies associated with clinical approach to counseling, have allowed handling the difficulties of successfully breastfeeding, reducing weaning in this group of infants. The complex lesions are multiple, difficult to access, extensive or affect the coronary arteries. Purpose - To report a case of complex cardiac injury. Case Report - RNBDS, 50, male, which regards self-flagellation with stab wound (knife) in precordial region initially stable and unchanged for laboratory tests, presented after 10 hours of the event, the revaluation: (FAST) with large amount of free fluid in the pericardial sac and jugular venous distension. Underwent pericardiocentesis and later left anterolateral thoracotomy, pericardiotomy and suture of 3 heart lesions. Conclusion - The resolution of buffering is just the first step in a grueling battle. Complex heart lesions are major problems and have a high mortality rate, even in experienced hands, because usually these are extremely violent and deadly bouts. The subtype of non-Hodgkin's lymphoma (NHL) mentioned in this report, there is more frequent involvement of multiple peripheral nodes, dissemination noncontiguous where the Waldeyer's ring and mesenteric lymph nodes are usually involved. The Mantle cell lymphoma (CML) is a lymphoproliferative disease of mature B-cell aggressive character corresponding to approximately 6% of all LNH4. This is a neoplasm with little chance of cure, representing an average of 25% survival at five years5. Objective and case report describe an atypical case of non- Hodgkin's lymphoma Mantle cell, affecting patients under 60 years of age, presence of B symptoms, referred to the otolaryngology Hosp Foundation. Adriano Jorge, reporting from its symptoms to definitive diagnosis. Alerting for the presumptive diagnosis of this condition so that in future demonstrations, easier recognition and treatment. Conclusion: The determination of prognostic index allowed to stratify the severity of the risk of mortality. However, despite the availability of advanced diagnostic methods lymphoma represents, for physicians and pathologists, a challenge regarding diagnosis and prognosis.
